This may be a 1:1 support model, 24 hours on site support or tailored to specific times of day. We will work with you and your loved ones to develop a personalised support plan focusing on your wishes, interests and support needs. This may include your personal care needs, management of your finances or even ways to be involved with local activities that you and your loved ones enjoy. You will have a key worker who will work with you day to day to achieve your personal goals, we will track your progress and continue to ensure we’re meeting your needs and that you’re happy with our service.
